Lifelong AIDS Alliance is a non-profit agency that is committed to preventing the spread of HIV, and to providing practical support services and advocating for those whose lives are affected by HIV and AIDS.
DEFINITION:
The Case Manager assists clients to access and engage in psychosocial, medical, home-care and supportive services. This position conducts comprehensive assessments, develops and updates service plans and facilitates the acquisition of all appropriate services. The Case Manager reports to the Case Management Supervisor. This is a full-time, exempt, union, regular position.
CORE RESPONSIBILITIES:
1. Conduct comprehensive assessments for people with HIV disease; develop, implement and revise service plans on an ongoing basis.
2. Link people to and serve as liaison between clients and public and private services.
3. Provide assistance with the acquisition of public assistance, Medicaid, food stamps, Social Security and other financial benefits and entitlements.
4. Provide assistance with insurance issues to ensure access to quality medical care.
5. Work with discharge planners in hospitals and Skilled Nursing Facilities to arrange for the services a client will need upon discharge.
6. Maintain and update client records and notes, including client tracking; required information reporting; progress notes; and other client documentation according to departmental standards while utilizing a computerized Case Management information system.
7. Attend and participate in team and all-staff meetings.
8. Work collaboratively with other AIDS Service Organizations, as well as Mental Health and Chemical Dependency Programs to ensure adequate care for clients.
9. Be intimately familiar with appropriate sources of referral for home care services, mental health and chemical dependency services, financial, housing, food, emotional support, etc., for people with HIV disease.
10. Engage in case management outreach activities.
11. Perform all assigned duties of position.

9. Maintain confidentiality of financial, donor and client records.
QUALIFICATIONS:
• Experience in advocacy services, case management or similar with African-American, Latino/a and /or other culturally diverse populations desired.
• Bachelor’s degree.
• Two years of HIV case management experience or three or more years of relevant case management experience.
• Master’s degree in a social service field is desirable.
• Three or more years of relevant experience.
• Two years of experience must be related to advocacy services, case management or similar work with the public in a direct service setting.
• Able to organize, prioritize multiple projects and meet deadlines in a time-sensitive environment.
• Possess an understanding of professional boundaries.
• Demonstrated skills and strong working knowledge of Microsoft Office suite of applications, including Excel, Access and Word.
• Experience working with Provide or similar database is a plus.
• Ability to communicate clearly and professionally in both verbal and written form.
• Proven ability to work independently and in a collaborative team environment.
• Experience with group facilitation, conflict resolution and a working knowledge of organizational development issues.
• Ability to respond quickly to change.
• Ability to develop good working relationships with internal and external customers.
• Demonstrated ability to work effectively under stressful conditions.
• Access to regular, dependable and timely transportation.
