Welcome Home Meal Delivery Program

Giving people their best chance at staying healthy at home

The days and weeks following a hospital stay are crucial to a person’s recovery. When you are faced with such challenges as lack of family or friends nearby, little to no income, limited mobility, and are living with a persistent, chronic illness — getting better seems so far out of reach.

The first of its kind in the Pacific Northwest, Lifelong’s Welcome Home Meal Delivery Program provides low-income, isolated individuals living with chronic illness with a series of nutritious, tasty meals within 24 hours of release from the hospital. All meals follow guidelines set by the American Diabetes Association and American Heart Association. Meals are customized for people according to their medical diagnosis. For example, the Easy Digestion option is for clients in need of a diet that is gentler on the digestive system, and the Dialysis Care option is for clients in kidney failure. Meals are packaged to be easily reheated in a microwave or oven.

A growing body of research shows that medically appropriate food is effective in improving health. In 2013, nearly 18% of Medicare patients nationally who had been hospitalized were readmitted within a month.

Sometimes something as simple as the right planning for a nutritional diet can make all the difference in a person’s health.

Program Details
The Welcome Home Meal Delivery Program consists of the following services:

  1. Your doctor, social worker, or family member calls to let us know that you are coming home from the hospital. You can call us, too!
  2. Within 24 hours of an individual being released from the hospital or nursing facility, you will receive a “Welcome Home”package consisting of 10 nutritious meals specialized for an individual’s unique medical diagnosis. The package also includes preparation instructions, a handy booklet prepared by registered dieticians on how to recover as quickly as you can from your illness, plus a brief questionnaire that will help us figure out how safe and comfortable you are.  
  3. A Lifelong care advocate will call you ever few days to check on you. They may ask if you’ve run into any problems with taking your meds on time or if you need help getting around.

Eligibility Requirements
You must be living in King County and be coming home from the hospital or nursing facility. Eligibility for services is contingent upon verification of disability due to illness, injury or age. There are no income requirements – we just want to make sure you’re OK.

How to Apply
We accept referrals from hospitals, emergency responders, or assisted living facilities. Our goal is to get you food and get you food quickly. Give us a call and we’ll figure out the rest. You can reach us at (206) 957-1686 or send us a message.

Delivery Information
We ask clients to be home when their delivery arrives. We will schedule a time that is convenient for you.  If you are unable to be home during that time, give us a call.
