Our case managers work one-on-one with people living with HIV/AIDS to make sure that no one who needs our help gets left behind. We want to educate and empower the people we serve to help them become self sufficient.
Our case managers have four primary functions:
• Assessment. The needs and goals for each client are identified through an in-depth interview.
• Planning. A “contract” between the client and case manager is developed that describes the steps necessary to meet those goals.
• Referral. Our case manager will refer the client to services and empower the client to practice the skills that will increase their independence.
• Monitoring. Our client and case manager will maintain enough contact to ensure that referrals continue to meet the client’s needs.
Case managers are carefully trained to help clients navigate through community resources for medical and dental care, health insurance programs, financial resources, housing and other support services.
Following a screening with one of our I & R Case Managers, you may be referred to a case manager who works with your medical provider, in your local community or you may be referred to a case manager here at Lifelong to complete an assessment and to create a “contract” for services.
Only those individuals that require ongoing case management assistance will be referred to a Lifelong case manager for a comprehensive assessment. If you are eligible for case management services here at Lifelong, but not referred for a comprehensive assessment, you will continue to work with our I & R case managers to meet your needs as they arise. If your need are more on-going you will be referred to an intensive case manager for a comprehensive assessment.
What is a comprehensive assessment?
The comprehensive assessment is an in-depth interview that typically takes two hours to complete. During the interview, we will discuss your health, any problems with adherence to medications, dental, insurance, finances, employment, housing, transportation, social/emotional health, mental health, chemical dependency, legal and several other areas.
What is the Service Plan Agreement?
At the completion of the comprehensive assessment, you and your case manager will create a Service Plan agreement. This agreement functions as a “contract” between the two of you. It will identify how you and your case manager can best achieve your goals together.
